Medical Module

Part of our PAM (Parent Access Module) student database set-up includes the medical profile of your child. You will be required to fill in these details, which can then be accessed by our school nurses, staff and, in the case of emergencies, the ambulance driver.

Information will be given to you on how to access this Medical Module. As part of our Duty of Care towards all students attending our school, we will require you to complete the Medical Module prior to your child attending the Transition Day in December.

A letter will be sent out to provide parents with their individual PAM login details and password.  All current families with a new student starting in 2020 will find their new student already on their PAM account.

Once login details are received please login and complete the Medical Module as soon as possible before Transition Day.

How to Access the Medical Module in PAM

Medical details for all Year 7 2020 students MUST be completed prior to Orientation Day on Monday, 2 December.

For the safety and wellbeing of your child, this is a duty of care and ensures that staff on campus have access to this information.  This information is then used when your child is on excursions or off campus for any school activities whilst a student at Assumption.

To access the Medical Module on PAM, you will need to first login to PAM. For all new families, you will be emailed your login details shortly. For current families with a new student starting in 2020, your child will be added to your current PAM account.


Once logged into PAM

  • Please verify your email address first.
  • Then click on ‘show inactive students’ if your student isn’t currently showing.
  • Click on your 2020 new student
  • Under “Personal Details”, click on ‘Medical Profile’
  • Please complete this in its entirety, as this cannot be saved otherwise.

Transition for students completing VCE:

Beginning Friday, 22 November and concluding Friday, 29 November.


Monday, 2 December, 2019

Start at 9.00 am in the Gonzales Centre, conclude at 3.00 pm in the Gonzales Centre. Students to wear primary school sports uniform and bring a pencil case and morning tea/lunch.

First day:

Thursday, January 30, 2020 – for Year 7, Year 12 and all new students

  • Year 7 students gather in the Gonzales Centre
  • New Year 8-11 students gather at the bus loop
  • Year 12 students attend normal Learning Mentor Groups

Friday, January 31, 2020 – for Year 8-11 students.

  • 8.45am:  All students attend their Learning Mentor Groups
